Comments:
A complaint inspection was initiated on September 23, 2022 and concluded on December 7, 2022 pertaining to allegations of staffing and supervision, physical plant, and special care provisions and emergencies. During this inspection, the licensing inspector conducted Interviews with staff, and collaterals, completed observations, took photographs, and reviewed staff/children records. There was a preponderance of evidence gathered in which the complaint was deemed valid for the concerns of staffing and supervision and physical plant. However, the allegation related to special care provisions and emergencies was not valid. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program inspection. Description: Based on review of staff record, the center did not ensure all staff completed a sworn statement or affirmation.

Evidence: In Staff#1's record (date of hire: 05/2022), there was no documentation of a sworn statement or affirmation found.

Plan of Correction: Staff no longer here, but all staff will have a sworn statement.

Description: Based on review of staff record, the center did not ensure to obtain central registry finding within 30 days of employment or volunteer service.

Evidence: In Staff#1's record (date of hire: 05/31/2022), there was no central registry findings found.

Complaint related: No
Description: Based on observation, interview with staff, photographs, the center did not ensure areas and equipment of the center, inside and outside are maintained in a clean, safe, and operable condition.

Evidence: During the inspection on September 23, 2022, there was a foul odor in the Threes/Fours restroom area and the following observations were made:
1. There was one toilet with smeared dried fecal matter on the seat and brownish circular buildup in toilet bowl.
2. There were three toilets with uncovered protruding screws exposed with visible rusting and brownish circular buildup in the toilet bowl.
3. The toilets in hallway nearby the school age area had uncovered protruding screws with visible rusting.
4. Interview with staff confirmed the conditions of the center's restrooms.
5. Photographs taken by the inspector during the inspection.

Plan of Correction: Staff will keep bathrooms and bathroom clean and safe.

Complaint related: No
Description: Based on interviews, the center did not ensure when supervising children to always ensure their care, protection, and guidance.

Evidence: 1. Interview with Staff#1 revealed an incident on or before July 14, 2022 involving Child#1 and Child#2, ages 10 &11 observed in the school age area under covers on the couch moving in back and forth motion.

2. Staff#1 confirmed not being aware of what the children in care were doing for approximately 10 minutes while assisting another child with an activity in a separate area.

3. Interviews with Staff #2, Staff#3, and Staff#4 acknowledged the incident occurring at the center.

Plan of Correction: All children will be in sight and sound supervision.
