An unannounced monitoring inspection was conducted on 11/28/2022 from 11:00am to 12:45pm. There were 10 children ages 3 years old to 5 years old supervised by 3 staff. The physical plant, outdoor playground area, programming, attendance, menus, 4 staff records, 3 children?s records, emergency drills, injury reports, and emergency supplies were reviewed. There were no children?s medications onsite. Children were observed participating in math and literacy activities. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-330-B Description: Based on observation and inspection of the playground, the licensee did not to ensure that the required amount of resilient surfacing was under playground equipment with moving parts or climbing apparatus to create a sufficient fall zone.
Evidence:
1)The slide was 6 feet tall and had 1 inch of wood chips in the surrounding fall zone instead of the required 6 inches of wood chips.
2)The swings were 105 inches high so they require 210 inches of fall zone in both the front of the swings and in the back of the swings at a depth of 6 inches of wood chips. The back fall zone of the swings contained 1 inch of wood chips for the first 177 inches to the border edge of the playground and contained no resilient surfacing beyond the border edge at 177 inches to 210 inches of the back of the swing fall zone. The front fall zone of the swings contained 1 inch of wood chips for the required 210 inches of fall zone in front of the swings.
3)The climber was 6 feet tall and had 1 inch of wood chips in the surrounding fall zone instead of the required 6 inches of wood chips.
4)The ladder was 6 feet tall and had 1 inch of wood chips in the surrounding fall zone instead of the required 6 inches of wood chips.
5)The 2 spring toys were 18 inches tall and had 1 inch of wood chips in the surrounding fall zone instead of the required 6 inches of wood chips.Plan of Correction: We have ordered new mulch. Will remain off playground until remedied.
Standard #: 8VAC20-780-530-A Description: Based on documentation review, the center did not have at least one staff member in each classroom or area where children are present that had obtained first aid and cardiopulmonary resuscitation (CPR) certification.
Evidence: In the Snapdragons classroom, Staff #2?s CPR certification expired on 8/29/2022 and there were no other staff in the classroom that had current CPR certification.Plan of Correction: We have scheduled a CPR and first aid training for all staff.
Standard #: 8VAC20-780-540-E Description: Based on observation and staff interview, the center did not have one working battery-operated radio in each building used by children.
Evidence: There was not a working battery operated radio in the building during the inspection.Plan of Correction: We ordered and already received a new one.
