Comments:
An unannounced monitoring inspection was conducted on December 12, 2022. There were 59 children present with 15 staff supervising. The physical plant, children's records, staff records, attendance records, training records, menu's, first aid kits and procedures for emergency preparedness were reviewed. The children were observed singing songs, preparing for holiday presentations and working in small groups. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
1. Child #1, #2 and #3's records do not contain a work phone number of each parent who has custody.
2. Child #1 and #2's records do not contain documentation of the names of person authorized to pick up the child.

Plan of Correction: I will contact the parents and once I have the information, it will be placed in the child's record.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center inside and outside shall be maintained in a clean, safe and operable conditions. Unsafe conditions shall include but not be limited to , splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment.

Evidence:
In the Nursery and the Active Asteroids classrooms, the changing tables have peeling paint.

Plan of Correction: I will contact the owner and have issue corrected.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Kitchen an unlocked closet contained a chemical glass cleaner.
2. In the Active Asteroids classroom, Bleach/water and Lysol were not in a locked cabinet/

Plan of Correction: The chemicals were removed from the closet and placed in a locked area during the time the inspector was on site.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center failed to ensure that if diaper ointment or cream is used, these products shall be in the original container and labeled with the child's name.

Evidence:
1. In the Active Asteroid classroom, there was one unlabeled container of diaper cream.
2. In the Rocking Robot classroom, there was one unlabeled container of diaper cream.

Plan of Correction: I will have teacher's label the diaper creams today.

Standard #: 8VAC20-780-550-C
Description: Based on observation, the center failed to ensure that the emergency preparedness plan will be posted in a location conspicuous to staff and children on each floor of the building.

Evidence:
The Emergency Preparedness procedures were not posted on each floor of the building.

Plan of Correction: The Emergency Procedures were posted while the inspector was on site.
