Technical Assistance:
Discussed the summary page and violation page and the importance of both pages being visible.

Comments:
An unannounced monitoring inspection was conducted on 12/08/2022 from 9:40am to 12:48pm. At the time of entrance, 88 children were in care with 18 staff members present. Children were observed during diaper changes, washing hands, playing on the floor with toys, coloring with markers, circle time activities, listening to music ?good morning song,? playing with toys at the table, outside play, listening to the teacher read a book, creative movement in the fitness class, practicing for a winter concert and singing songs, sorting objects, working on an art project and lining up to go outside as the teacher did a head count and name to face count. Interactions between the children and staff were positive. The site was organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, medications, the physical space, evacuation drills and attendance records were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of two staff records, the center did not have documentation of a tuberculosis (TB) test or screening at the time of employment and prior to coming into contact with children that was completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #1 (date of hire 11/14/2022) had a TB test or screening dated 11/28/2022.
2. Staff #2 (date of hire 11/15/2022) had a TB test or screening dated 11/17/2022.

Plan of Correction: Before starting, both staff stated they would obtain TB screenings by their
start date, but then became delayed due to scheduling issues. They
started training due to our staffing shortage and have since complied
with this requirement. [Staff #3 (not #2 as indicated in violation) started on
11/15.] Moving forward, we will obtain TB results prior to teachers
starting their training with us.

Standard #: 8VAC20-780-70
Description: Based on record review, staff records did not include all required information.
Evidence:
1. Staff #2 (date of hire 10/24/2022) and Staff #5 (date of hire 10/24/2022) did not have documentation that the individual possesses the education required by the job position.

Plan of Correction: Both teachers have met the educational requirement for their job position but had
difficulty finding their diplomas, so they contacted their respective schools for a
transcript. They started working on their start date due to our staffing shortage and have committed to meeting this requirement immediately. Moving forward, we will obtain documentation by the start date.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas of the center, inside were not maintained in a safe and operable condition.
Evidence:
1. In the Giraffes classroom, the paint was chipping by the window near the play kitchen set.

Plan of Correction: We will contact our maintenance person to re-paint that area of the
wall. We will also remind teachers at our next staff meeting on 12/15 to
immediately report any repairs needed.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to clearly date food containers when food is brought from home.
Evidence:
1. In the Panda classroom, two lunch boxes were not labeled with the date.
2. In the Whales classroom, three lunch boxes were not labeled with the date.

Plan of Correction: Teachers were reminded immediately about this requirement
and corrected it. All teachers will be reminded at our upcoming staff
meeting on 12/15.
