The LI showed staff where to find model forms and showed them certain features of the model form for new staff orientation.
- Comments:

An unannounced monitoring inspection was conducted on 12/6/2022 beginning at 11am and concluding at 1:40pm. Upon arrival there were a total of 46 children, ages 3 years to 5 years, under the care of 9 staff. The Licensing Inspector reviewed three children?s records and three staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. The children were observed during free play, handwashing, lunch, and nap time.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: Based on record review, the center failed to obtain a repeat background check for an employee. Repeat CPS (Central Registry) checks must be repeated every five years.
