Comments:
An unannounced on-site renewal inspection was conducted on 12/8/2022. There were 36 children, ages 4 months-5 years, and 10 staff members present during the inspection. The inspector observed the following: arrival, breakfast, free choice lunch, circle time and nap. A total of 5 children?s records and 7 staff records and 4 board officers, were reviewed. The inspector arrived at 9:00 AM and departed the program at 11:20 AM

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of four Board Officer?s records, the center failed to obtain the finding from a search of the central registry within 30 days of taking office.

Evidence:

1. Board Officer # 1 and # 2 took office in September 2022. A return search of the central registry was not in the record. Staff # 8 did provide documentation that the CPS request was sent 11/14/2022.

Plan of Correction: The center as submitted the CPS request, in the future the CPS checks will be done on the portal and will be done at the time the board officer takes office.

Standard #: 8VAC20-780-140-A
Description: Based on review of five children's records, the center failed to ensure that all children had a physical examination within 30 days of enrollment.
Evidence:

1.The record for Child # 2, started on 2/18/2022, physical exam within 30 days of beginning attendance. The date on the physical exam was 4/22/2022.

Plan of Correction: In the future all physicals will be obtained in the first 30 days of enrollment.

Standard #: 8VAC20-780-60-A
Description: Based on review of eight children?s records, the center failed to ensure that all records were complete per the standard.

Evidence:
1. The record for Child # 2 was missing previous child care and other programs, and chronic physical problems and allergies.
2. The record for Child # 3 was the address for one of the two emergency contacts.

Plan of Correction: The director will have the parents to update the enrollment form and will ensure they are complete.

Standard #: 8VAC20-780-280-B
Description: Based on observation of the two year old classroom room, the center failed to ensure all chemicals were locked.

Evidence:
1.The inspector observed several containers of chemicals in an unlocked cabinet under the diaper changing area. The children have access to the area and the chemicals were accessible.

Plan of Correction: The teacher locked the cabinet during the inspection and will make sure that it remains off when chemicals are not being used.
