Technical Assistance:

Blue rainbow climber on the playground is not to be used until secured into the ground properly.

Comments:

An unannounced Monitoring Inspection took place on 12/08/2022 between the hours of approximately 11:30 a.m. and 12:50 p.m. There were 3 classrooms observed with a total of 13 children with 4 staff within the supervision guidelines. The children were observed preparing to go to the playground, engaging in free play and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, and fire drill log were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:

Standard #:	8VAC20-780-270-A
Description:	Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition. Evidence: 1. In the school age room, there was a chipped and cracked tile on the floor near the computers. 2. On the playground, there was a blue rainbow climber that was not securely anchored to the ground. When pulling on the structure, the structure began to tilt over. 3. On the playground, there was chipped paint and rust on the bars of a yellow, metal car structure. 4. On the playground, the end cap for the left handlebar of a yellow bike was missing. The rubber covering for the front tire was also missing. 5. On the playground, there was a wooden pole attached to a slab of concrete lying in the grassy area. 6. On the playground, there was a large orange cone that was easy to tip over, in the grassy area, accessible to children. 7. In the infant classroom, there was a crockpot with hot water inside, located on the floor by the refrigerator, accessible to children. 8. In the toddler classroom, there was a hole in the bathroom door, within reach of children.
Plan of Correction:	The doors and floor will be fixed by next weekend. The rainbow climber will be anchored by the end of the month. I will move the crockpot out of reach and will purchase a bottle warmer.

Description:	Based on observation, resilient surfacing did not comply with the minimum safety standards. Evidence: 1. The mulch located in the fall zone around the blue rainbow climber was worn down to dirt in multiple areas. 2. The mulch located at the bottom of the blue slide was worn down to dirt.
Plan of Correction:	A work order to replace the mulch was submitted on 11/30. We will have it replaced by the end of the month.
