Comments:
An unannounced monitoring inspection was conducted on-site on 12/12/2022. There were 70 children, ages 2 years-5 years, 13 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed during meal time, child choice activities, and outdoor time. The inspector discussed the following with the provider: playground and emergency drills. A total of 5 children?s records, 5 staff records and 1 medication were reviewed. The inspector arrived for the inspection at 9:30 am and departed at 12:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and the documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence: The record for Staff 2 contained a documented hire date of 10/1/2022. The date on the tuberculosis screening in the record was 10/07/2021.

Plan of Correction: Moving forward, staff will obtain information at time of hire by 12/13/2022.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and interviews with staff, the center failed to ensure a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record review and medication review for Child 1 indicated a diagnosed food allergy. Staff 6 confirmed no allergy care plan was available.

Plan of Correction: Staff will obtain information from family by 12/16/2022.
