Discussed with center Director diapering mat and emailed orientation training model form.

Comments:
An unannounced monitoring inspection was conducted today from 9:20am-10:45am. There were 32 children (infants-4yrs.) directly supervised by 6 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, reading books and napping. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Description: Based on record review, the center did not obtain a Tuberculosis (TB) screening or test at least every two years from the date of the last screening /test.

Evidence:
Staff #4's record contains a TB screening/test that was last updated on 7/10/2020.

Description: Based on record review, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Staff #2's record does not contain documentation that two or more references were checked before employment.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the center did not ensure that staff completed the Virginia Department of Education-sponsored orientation course within 90 days of employment.

Evidence:
The records of Staff #1 (DOH:7/8/2019), Staff #2 (DOH:8/16/2022) and Staff #3 (DOH 11/10/2021) did not contain the Virginia department of Education-sponsored orientation training certificate.

Description: Based on record review and interview, the center did not ensure there was at least one staff on duty who has obtained within the last three years instruction in performing a daily health observation of children.

Evidence:
There wasn't a staff person present during the inspection that had received instruction in performing a daily health observation of children within the last three years.

Plan of Correction: I'm locating online training for 2 staff members.

Description: Based on observation, prepared infant formula was not dated and labeled with the child's name.

Evidence:
In the Infant B classroom, 4 bottles of prepared infant formula were observed in the classroom's refrigerator not labeled with the child's name and the date.

Plan of Correction: Corrected on the spot December 8, 2022.
