Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program.
An initial monitoring inspection was initiated and concluded on December 13, 2022.
The inspector reviewed compliance in the areas of administration, physical plant, programming, medication, special care and emergencies and nutrition. A total of 1 staff record and 2 other persons background checks were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-K
Description: Based on review of center documentation and staff interviews, the licensee did not ensure the center shall develop written procedures for abusive head trauma.

Evidence: Staff #1 confirmed there was not a policy available for review related to head trauma.

Plan of Correction: Director will write up policy for Abusive Head Trauma- corrected on 12/19/ 20- 22

Standard #: 8VAC20-780-40-N
Description: Based on review of center documentation and staff interviews, the licensee did not ensure to develop written procedures for playground safety procedures that shall include, provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children.

Evidence: Staff #1 confirmed there was not a written playground safety procedure available for review.

Plan of Correction: Director will write up a plan of action procedures for playground safety and include provision for playground safety and include provision for active supervision by staff, scanning and circulating- corrected on 12/19/2022

Standard #: 8VAC20-780-320-B
Description: Based on observation and staff interviews, the licensee did not ensure to have all required items for restrooms.

Evidence: The restroom between the toddler room and the preschool room did not have the following as required;
1.The toilet did not flush.
2.The sink located near the toilet did not have warm running water

Plan of Correction: Director called Facilities manager at school and Facilities manager called plumber who came and corrected t oilet. Toilet in bathroom between the 2 classrooms now flushes correctly. Plumber also adjusted water temperature to (no more than 120 degrees) in said bathroom and the other bathroom used by the school

Standard #: 8VAC20-780-550-B
Description: Based on review of center documentation and staff interviews, the licensee did not ensure the center shall develop written procedures for an emergency preparedness plan that contains all required procedural components. .
Evidence: Staff #1 confirmed the following procedural component were not included in the emergency preparedness plan. 1) Accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation, relocation or lockdown.
2) Procedures to reunite children with a parent or authorized person designated by the parent to pick up the child.
3)Continuity of operations to ensure that essential functions are maintained during an emergency.
4) Staff training requirement, drill frequency, and plan review and update.

Plan of Correction: Director will write up procedure for emergency preparedness to include Accommodations for special needs for infants and toddlers to ensure their safety during evacuation and lockdown. Director will also write out procedures to reunite children with their parents/authorized person to pick-up their child and write up continuity of operations to ensure that essential functions are maintained during an emergency. Director will also write up and update staff training and requirement/drill frequency plan-This is corrected on 12/19/2022
