Technical Assistance:
Staff 1 requested some additional training in record keeping. The LI will set up a time to do this after the upcoming holidays.

Comments:
An unannounced monitoring inspection was conducted on 12/12/2022 from 2:50pm to 5:50pm. The LI arrived before the children did, but there was a total of 19 children, ages 6 years to 12 years, in three groups under the care of three staff after they finished arriving. The Licensing Inspector reviewed three children?s records and four staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. The children were observed eating, doing homework, and in free play time.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and interview with staff, the center failed to ensure that the fingerprint criminal record check did not include a disqualifying background.

EVIDENCE:

Staff 3 had a documented hire date of 9/20/2017. Documentation in the record showed that Staff 3 was convicted of a barrier crime on 6/27/2018. The fingerprint record result letter dated January 22, 2019 had a ?NOT ELIGIBLE? result for Staff 3. Another fingerprint record result letter dated January 21, 2022 had a ?NOT ELIGIBLE? result for Staff 3. Staff 5 stated that center management was aware of the barrier crime conviction but continued the employment of Staff 3.

Plan of Correction: Staff 3 was transferred to Adult Services in another building. (STEM Center) He is no longer on the property where children are located during the after-school tutorial program.

Standard #: 22.1-289.035-B-2
Description: Based on record review, the center failed to ensure that the fingerprint criminal record check was completed on a staff person prior to hire as required.

EVIDENCE:

The record for Staff 4 did not have an eligibility letter from the Office of Background Investigation-Criminal Background Unit. Staff 4 was observed working in the center on the day of the inspection. Staff 4 stated they had been working since September 2022 but the exact start date was not recalled or in the record.

Plan of Correction: All regular staff, interns, JFDC board officers, and Work-study volunteer staff will not be released to work/operate at the Jubilee Center until all background checks have been cleared, orientation reviewed, TB screening results, reference checks completed, trainings, and other required forms have been signed and reviewed by the Office Manager, and Program Director respectively.

Description: Based on record review and interviews, the center failed to have some required information in a staff record.

EVIDENCE:

The record for Staff 4 did not have a start date. Staff 4 stated they had been working since September 2022 but the exact start date was not recalled. Staff 1 stated Staff 4 has been working since approximately September 2022.

Plan of Correction: Staff 4 -We have corrected the record to show his actual starting date. From now on we will have all staff start dates and release dates notated on their applications prior to starting at the Center.

Standard #: 8VAC20-780-240-B
Description: Based on record review and interview, the center failed to provide new staff orientation no later than seven days of the date of assuming job responsibilities.

EVIDENCE:
Staff 4 began work at the center sometime in September 2022 (exact date unknown). The record for Staff 4 did not have documentation to support that new staff orientation was done.

Plan of Correction: Staff 4 was given Jubilee Orientation and SOP for staff

Standard #: 8VAC20-780-340-D
Description: Based on observation and record review, the center failed to ensure there was a person qualified as a program leader in each group of children.

EVIDENCE:

Staff 4 was observed acting as a program leader in a group of six children. The record for Staff 4 did not have documentation to support they are qualified to be a program leader.

Plan of Correction: A qualified staff member shall be present in any group of children at the center at all times.

Standard #: 8VAC20-780-530-A
Description: Based on observation and record review, the center failed to ensure that there was a staff in each area where children were present who was currently certified in first aid and CPR.

EVIDENCE:

Staff 4 was observed as being the only staff person with a group of children. Staff 4 did not have documentation of current first aid and CPR certification in the record.

Plan of Correction: All Jubilee staff and Work-study volunteers will be trained in First Aid, AED, Bloodborne Pathogens within 60 days of hire date. Staff 4 was trained and certified in First Aid, CPR, AED, BB Pathogens.
