An unannounced monitoring visit and facility tour was conducted on 12/6/2022. At the time of inspectors' arrival there were 24 infant to preschool aged children in care with 3 staff members and 1 administrators. Children were observed interacting with staff, and engaging in free play and educational activities. A sample of 5 children's records and 4 staff records were reviewed. Assistant Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-780-140-A Description: Based on record review and interview it was determined the center did not ensure each child had a physical examination by or under the direction of a physician.
Evidence:
1. The records for child #2, 3, 4, and 5 did not contain documentation of a physical exam.
2. Staff confirmed these records were not available duding this inspection.Plan of Correction: Director will request documentation from parents.
Standard #: 8VAC20-780-160-A Description: Based on observation it was determined the each staff member did not have documentation of a negative tuberculosis screening.
Evidence:
1. The record for staff #4 did not contain documentation of a TB screening.
2. Staff confirmed they could not locate this documentation during this inspection.Plan of Correction: Staff will get documentation of a TB screening.
Standard #: 8VAC20-780-340-F Description: Based on observation and interview it was determined the center did not ensure that children under the age of 10 years were always within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.
Evidence:
1. Inspector observed child #4(3yrs 8mos old) walk into the restroom without supervision and then walk from the restroom down the hall into the foyer and then into the classroom without supervision of staff.
2. Staff confirmed this occurred.Plan of Correction: Staff will ensure each child is in sight and sound supervision while in care.
Standard #: 8VAC20-780-350-B-1 Description: Based on observation it was determined the center did not ensure that the 1:4 staff:children ratio was met for children under 16 months of age.
Evidence:
1. There was 5 children aged 6 mos to 2 years in care with one staff member.
2. Staff confirmed that they were out of ratio.Plan of Correction: Center is looking to hire more staff.
Standard #: 8VAC20-780-350-B-4 Description: Based on observation and interview it was determined the center did not ensure the ration of 1:10 staff:children was met for 3 year olds to school aged.
Evidence:
1. There were eleven 3-4 year olds in care with 1 staff person.
2. Staff confirmed that they were out of ratio.Plan of Correction: Center is looking to hire more staff.
