1) 40M - maintain a current written list of all children's allergies, sensitivities and dietary restrictions. This list shall be kept in each room or area where children are present, including the playground, gym, bus/van
2) 500B3 - A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing
3) 500B7 - The diapering surface shall be used only for diapering

An unannounced monitoring inspection was conducted from 10:30 am - 2:30 pm with the center director. There were 76 children, ranging from two months to five years of age, present with 15 staff supervising. Children were observed playing with age-appropriate toys in a variety of centers, participating in circle time and arts and crafts activities, outside playing on the playground, and washing hands preparing for lunch time. Lunch served today: fish sticks, whole wheat bread, corn, pears and milk. Five child records and five staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, one medication, and the annual fire and health inspections. Standard #: 22.1-289.035-B-4 Description: Based on review of five staff records, it was determined that out of state background checks were not completed as required. Evidence: The record for staff #5, (date of hire: 9/21/2022) who indicated they lived out of state in the past five years, did not contain documentation that an Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment/follow up completed or an Out Of State Criminal History Name Check prior to employment for all states indicated on the sworn disclosure statement. Standard #: 8VAC20-780-80-A Description: Based on review of the written attendance record, and interviews with staff, it was determined that the facility did not ensure that the written record of daily attendance, for each group of children, documents the arrival and departure of all children in care as it occurs. Evidence: 1) in the infant B class, four children were observed, however five children were listed on the attendance sheet; 2) in the pre-k A class, 10 children were observed, however 11 children were listed on the attendance sheet; 3) in the pre-k B class, 13 children were observed, however 14 children were listed on the attendance sheet. Standard #: 8VAC20-780-270-A Description: Based on observation, it was determined that not all areas inside the center were maintained in a safe and operable condition. Evidence: there was a crib with loose fitting sheets on the mattress in both the infant A and infant B classroom.
