An unannounced monitoring inspection was conducted on December 9, 2022 from approximately 10:45 am to 12:45 pm. All areas of the center were observed including classrooms, bathrooms, and the playground. There were 47 children in attendance and 11 staff. 5 children?s records and 6 staff records were reviewed. The children were observed playing with blocks, painting, listening to music, and eating lunch while the Licensing Inspector was on the premises. The center has appropriate and adequate materials for the children.
The most recent health inspection was done on 2/2/22. The most recent fire inspection was completed on 11/29/22. Evacuation drills were conducted monthly with the most recent completed on 11/28/22.
The first aid kit was inspected and found complete. The center had a working flashlight and radio.
Violations were cited as a result of this inspection.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 12/21/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 8VAC20-780-60-A Description: Based on records review, and interview, the center failed to maintain and keep at the center documentation of viewing proof of the child?s identity and age in 3 out of 5 records viewed. Evidence: The records for child #1, date of enrollment 10/3/22, child #2, date of enrollment, 7/5/22, and child # 3, date of enrollment 7/5/22, did not contain documentation of viewing proof of the child?s identity. When asked the Director stated that she did not have the documentation. Plan of Correction: Per the center "I will ask the parents for the documentation needed and put them in their files."
Standard #: 8VAC20-780-270-A Description: Based on observation the center failed to ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe, and operable condition. Evidence: The wooden fencing on the school age playground has several broken and/or missing slats. It also has exposed nails in several places and warped, rough wood. Plan of Correction: Per the center "replace the missing boards; hammer the exposed nails; replace or sand rough wood; replace warped boards."
