Comments:

An unannounced monitoring inspection was conducted on December 9, 2022 from approximately 4:00 pm to 5:00 pm. There were 22 children with 3 staff directly supervising. 3 staff records and 3 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. Children were observed playing in the gym while the licensing inspector was on the premises. The most recent health inspection was done on 10/14/22. The most recent fire inspection was completed on 7/6/22. Emergency drills were conducted monthly with the most recent completed on 11/21/22. The first aid kit was inspected and found complete. The center had a working flashlight and radio. Violations were cited as a result of this inspection. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 12/22/2022. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #:	8VAC20-770-60-C-2
Description:	Based on records review, the center failed to have a central registry search within 30 days of employment. Evidence: The record for staff #3, date of employment 5/20/22, did not contain documentation of the Central Registry search being submitted or received.
Plan of Correction:	Per the center "we have fixed our CRS records process for all new staff members. We added a new process with HR to confirm these are completed timely."
Standard #:	8VAC20-780-160-C
Description:	Based on records review, and interview, the center failed to ensure staff obtain and submit the results of a follow up tuberculosis screening at least every two years from the date of the initial screening or testing. Evidence: The record for staff #2, date of employment 9/4/20, contained a tuberculosis screening dated 9/2/20. When interviewed the Administrator stated that the follow up tuberculosis screening had not been completed.
Plan of Correction:	Per the center "we will make sure all staff complete required background and TB screens/testing. We have sent this staff person to get this redone."
Standard #:	8VAC20-780-240-A
Description:	Based on records review, and interview, the center failed to maintain documentation that staff have completed The Virginia Department of Education (VDOE)-sponsored orientation course within 90 calendar days of employment. Evidence: The record for staff #3, date of employment 5/20/22, did not contain documentation of the VDOE sponsored orientation course being completed. When interviewed, the Administrator stated that the documentation was not in the record.
Plan of Correction:	Per the center "we have sent details to this staff to correct and finish this training. All staff have 30 days to complete this training. We will continue our process and make sure this is followed."
Standard #:	8VAC20-780-240-E
Description:	Based on records review, and interview, the center failed to ensure that staff complete orientation training in first aid and CPR within the first 30 days of employment. Evidence: The record for staff #3, date of employment 5/20/22, did not contain documentation of orientation training in first aid and CPR. When asked, the Administrator stated that staff #3 had not completed the training.
Plan of Correction:	Per the center "our process is staff must complete within 30 days. We will make sure this process is followed. We have signed up staff for cpr/fa courses."
