Technical Assistance:

Discussed new standards review and ongoing cleaning of areas of the center.

Comments:

Today an unannounced Monitoring Inspection took place between the hours of 2:35pm and 4:05pm. There were 7 classrooms observed with a total of 50 children with 11 staff within the supervision guidelines. The children were observed playing card games, putting together puzzles, being held and fed bottles, playing outside on the playground and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard #:	22.1-289.035-B-4
Description:	Based on a review of staff records, 1 out of 4 staff did not have a criminal history record information check for a state in which the individual has resided in the preceding five years. Evidence: 1. Staff B did not have documentation of an out-of-state criminal history record information check for state #2 in which the individual has resided in the preceding five years at the time of the inspection.
Plan of Correction:	We will conduct the required out-of-state background checks on time moving forward and will send the staff member's criminal history check out as soon as possible.
Standard #:	8VAC20-770-60-B
Description:	Based on a review of staff records, 1 out of 4 staff was employed prior to completing the sworn statement or affirmation. Evidence: 1. Staff C (doh: 9/19/2022) has a sworn statement or affirmation on file that was dated on 9/22/2022.
Plan of Correction:	We will have staff sign their sworn statement on time moving forward.
Standard #:	8VAC20-780-160-A
Description:	Based on a review of staff records, 3 out of 4 staff did not submit documentation of a negative tuberculosis screening that was completed within the last 30 calendar days of the date of employment. Evidence: 1. Staff A (doh: 6/10/2022) had a tuberculosis screening on file dated 6/11/2022. 2. Staff B (doh: 6/8/2022) had a tuberculosis screening on file dated 1/12/2022. 3. Staff C (doh: 10/11/2022) had a tuberculosis screening on file dated 10/11/2022.
Plan of Correction:	We will get the TB screenings for staff on time moving forward.

Description:	Based on a review of staff records, 1 out of 4 staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment. Evidence: 1. Staff A (doh: 6/10/2022) did not have documentation of completing the Virginia Department of Education-sponsored orientation course on file at the time of the inspection. 2. Staff C (doh: 9/19/2022) did not have documentation of completing the Virginia Department of Education-sponsored orientation course on file at the time of the inspection.
Plan of Correction:	We will have the employees complete this as soon as possible.
