Comments:
A monitoring inspection was conducted on December 27, 2022, approximately 10:35 am to 12:00 noon. Licensing was greeted by the Assistant Director. A walk-though was conducted of the child day center. Licensing observed staff engaged and interactive with the children. Children were observed in group activity talking about the weather, individual play and center activity for colors.
There were 61 children and 14 staff during the inspection.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on staff records review, the child day center did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #4 (date of employment 10/17/2022) did not central registry results from North Carolina available for review during the inspection.
2. Staff #5 (date of employment 1/18/2011) criminal history results expired on 2/16/2022.
3. Staff #7 (date of employment 8/26/2010) central registry results expired on 2/07/2022 and sworn disclosure statement expired on 1/10/2022.

Plan of Correction: Staff were notified of expired paperwork. Waiting on results of paperwork.

Standard #: 8VAC20-780-160-C
Description: Based on staff records review, the center did not obtain the required documents for staff a tuberculosis screening at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. Staff #2 (date of employment 3/09/2020 tuberculosis screening expired on 11/16/2022.
2. Staff #5 (date of employment 1/18/2021) tuberculosis screening expired on 6/01/2018.
3. Staff #7 (date of employment 8/26/2010) tuberculosis screening expired on 11/17/2022.

Plan of Correction: Gave all 3 staff TB paperwork & will schedule appointment ASAP.

Standard #: 8VAC20-780-270-A
Description: Based on classroom observations, areas and equipment of the center inside were maintained in a safe.
Evidence:
1. In the classroom for 2's, 3's, and 4's, protruding screws were on the base of the toilet that were not covered by toilet caps. The bathrooms are accessible to children.

Plan of Correction: Contacted Liberty Plumbing and ordered new caps
