Technical Assistance:
*Discussed the new orientation form for staff - which lists changes to staff policies/procedures effective October 2021.
*Discussed possible safety issue with accessible pipe in upstairs play room. (occasionally in use)
*Clarified bathroom supervision for preschool age children - Standard 340.F.

Comments:
An unannounced monitoring inspection was conducted on 12/01/2022 from 10:05 am - 11:52 am. At the time of entrance there were 64 children in care with 11 teaching staff. The children in care ranged in age from two years - five years. Children were observed during morning program time and during outdoor play. Departure/carpool was additionally reviewed.
Records reviewed for three staff and four children in care.

Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on review and interview, the center failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities. This list shall be dated and kept confidential in each room or area where children are present.

Evidence:
1. The written allergy list was not available in every classroom where children were present.
a. The allergy list was not dated as required.
2. Administrative staff stated the allergy list was only in the classroom of the child with a diagnosed food allergy.

Plan of Correction: Administrative staff stated the allergy list will be dated and posted, confidentially, in each classroom.

Standard #: 8VAC20-780-80-A
Description: Based on review and interview, the center failed to maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The attendance record had not been completed in one classroom (pre-K) when reviewed at 11:25 am.
2. Classroom staff verified that attendance had not been completed.

Plan of Correction: Administrative staff stated that attendance will be documented as children arrive and depart going forward.

Standard #: 8VAC20-780-510-G
Description: Based on review, the center failed to ensure that medication shall be labeled with the child's name.

Evidence:
1. One over-the-counter medication for child 1 was not labeled with the child's name.
a. The medication was stored inside of a labeled bag.

Plan of Correction: Medication was labeled during the inspection.

Standard #: 8VAC20-820-120-E-2
Description: Based on review, the center failed to ensure that the findings of the most recent inspection of the facility shall be posted on the premises.

Evidence:
1. The findings of the most recent monitoring inspection conducted on 05/24/2022 were not posted.
a. The findings of the 03/11/2021 inspection were posted.

Plan of Correction: Administrative staff stated that current inspection reports will be posted.
