Comments:
An unannounced monitoring inspection was conducted on January 3, 2023. There were 85 children present with 12 staff supervising. The physical plant, menu's, attendance logs, evacuation drill logs, first aid kit, flashlights and radios, children's records, bottles and cribs and the emergency preparedness procedures were reviewed. There were no medications on site. Areas of non-compliance were cited on the violation notice. The children were observed engaging in a variety of educational activities, including alphabet recognition, number recognition and singing and reading stories. Description: Based on a review of staff records, the center failed to obtain at least every two years from the date of the initial screening or testing the results of a follow-up tuberculosis screening.

Description: Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #2, #3 and #4's records did not contain the work phone number and/or place of employment of each parent who has custody.
Child #2's record did not contain documentation of the addresses of two designated people to call in an emergency.
Child #1's record did not contain documentation of the names of person authorized to pick up the child in an emergency, if the parent can not be reached.

Description: Based on a review of staff records, the center failed to ensure that staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Description: Based on observation, the center failed to ensure that areas and equipment inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include but not be limited to, splintered, cracked or otherwise deteriorating wood, chipped or peeling paint, visible cracks, bending or warping, rusting or breakage of any equipment, head entrapment hazards, and protruding nails, bolts or other components that could entangle clothing or snag skin.

In the 3 year old classroom, a wooden top has been placed over the toilet tank and the wood is splintering and the paint is peeling.

Description: Based on observation, the center failed to ensure that hazardous substances, such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the bathroom connected to the Toddler classroom, cleaning products were not locked away to prevent access by children.
In the 4 year old classroom, cleaning products were not locked away to prevent access by children.

Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
