Technical Assistance discussed with Management:

-Requirements for fitted crib sheets as the center is planning for parents to provide them for Infants.
-The large playground will not be used by children until an inspection of the playground takes place, after construction is finished.
-Evidence of the Staff?s Education must be on record to qualify them as a Program Lead.
-Sounding of alarms and frequency for fire drills, shelter-in-place drills, and lockdown drills.
-A nonabsorbent surface for diapering or changing shall be used. For children younger than three-years-old, this surface shall be a changing table or countertop designated for changing.

Comments:
An announced initial inspection was conducted on 12/7/22 from 12:45pm to 3:10pm with the Owner, Director, and Assistant Director. The center initially plans to operate Monday through Saturday from 7:30am to 5:30pm. The center will care for up to 107 children from six-weeks-old to 12-years-old. Most, but not all, standards were reviewed as standards related to supervision, child records, and programming could not be observed because there were no children in care during the inspection. The physical plant was observed and measured to verify capacity. The emergency preparedness plans, parent handbook, staff handbook, and emergency supplies were reviewed at this time. Emergency numbers, evacuation maps/procedures and daily schedules were posted. Five staff records were reviewed, the Director?s record and Board Officer Background checks were reviewed previously. Medications will be administered upon parental request, appropriate authorizations, and in accordance with the center's medication policies and procedures. One staff has current certification in Medication Administration Training (MAT) and others will be getting certified. The Center has two staff with current certification in CPR and First Aid, and two staff with current Daily Health Observation Training (DHO). The most recent Fire Inspection submitted was dated 12/29/22 and the most recent Health Inspection on file was dated 11/30/22. Classrooms are equipped with a variety of age appropriate materials. Meals will be provided by the parents initially. A conditional (six-month) license will be recommended at this time. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not observed at the initial inspection. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 1/4/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas of the center, inside and outside, were maintained in a safe condition. Evidence: The fence separating the large playground from the small playground had openings that allowed a small torso template (3-1/2 inch by 6-1/4 inch rectangle) to fit through but not a large head template (9 inch circle) to fit through, which presented a head entrapment hazard.

Plan of Correction: The playground was under construction at the time of the inspection. Due to weather we had to wait on completion. We are going to change the fence to have the boards close together like the outside fence for compliance with licensing.

Standard #: 8VAC20-780-540-C
Description: Based on observation, the first aid kit did not include all of the required items. Evidence: The first aid kit did not include triangular bandages or a thermometer.

Plan of Correction: Items were purchased.

Standard #: 8VAC20-780-540-E
Description: Based on observation and interview, the center did not obtain all of the required nonmedical emergency supplies. Evidence: The managers reported that there was neither a working, battery-operated flashlight nor a working, battery-operated radio available at the center.
