Technical Assistance:
Technical assistance was offered related to: water bottles, medications and allergies, playground, child supervision records and staff orientation forms.

Comments:
A monitoring inspection was conducted on 1/4/23 from 9:30 am to 12:40 pm, in response to a self-reported incident that occurred on 12/14/22. The incident was related to inappropriate behavior guidance towards children in care. Various other areas of the standards were reviewed during the inspection as well. 2 staff members were interviewed, 4 staff records and 3 children's records were reviewed. Medications, the playground and other areas of center were reviewed. The children were observed engaged in outdoor play, circle time and structured small group activities. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice. Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child # 2's date of birth is 9/7/21.
2. Child # 2's most recent documentation of immunization was dated 4/8/22. Additional immunization records were not obtained 6 months from that date.

Plan of Correction: We have requested updated records to e submitted.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances were not maintain in locked place using a safe locking method that prevents access by children.
Evidence:
1. Cleaning spray bottles were stored in cupboards in the Preschool (3-year old) classroom and the 4's classroom.
2. The doors to both cupboards had a safety lock which unlocked as soon as the doors to the cupboards were opened.

Plan of Correction: The existing locks have been replaced with new safety locks.

Standard #: 8VAC20-780-350-B-2
Description: Based on records review, the ratio of 1 staff to 5 children was not maintained in a classroom with children ranging from 16 months to 24 months.
Evidence:
1. The Child Supervision Record (CSR) dated 1/4/23 for the Toddler room showed that there was only one staff present from 7:49 am to 8:30 am with 6 children.
2. The toddler room has children from 16 months to 24 months of age; and the ratio of 1 staff to 5 children was not maintained during the time mentioned above.

Plan of Correction: Schedules have been adjusted to maintain ratios in classrooms.

Standard #: 8VAC20-780-350-B-4
Description: Based on records review, the center did not maintain the ratio of 1 staff to 10 children in a four year old classroom.
Evidence:
1. The Child Supervision Record (CSR) dated 12/14/22 showed that Staff # 1 was by herself with a total of 11 children from 7:33 am to 7:45 am.
2. The 11 children included 6-four year old children and 5 children that were school agers (5 years and above), bringing the ratio of the classroom to 1 staff to 10 children.
3. Staff # 1 was over ratio from 7:33 am to 7:45 am.

Plan of Correction: We have adjusted schedules to maintain ratios.

Standard #: 8VAC20-780-400-B
Description: Based on staff interview, behavioral guidance was not constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
Evidence:
1. On 12/14/22, Staff # 1 was in the school age room with school agers that included Child # 1 (9 years old).
2. Staff # 1 stated that Child # 1 was asked to sit down for quiet reading time. Child # 1 did not listen to Staff # 1 and sat down on the floor in the right hand corner across from the entrance of the room.
3. Staff # 1 stated that as Child # 1 was sitting on the floor, he kept sliding down the wall onto the rug. After being asked to stop doing that, Child # 1 continued to do so.
4. Staff # 1 stated that she had a small book in her hand and "tapped" the book on Child # 1's left leg and asked him to stop.

Plan of Correction: A 30-day retraining plan has been put into place focusing in appropriate behavior guidance.
