Today an unannounced Renewal Inspection took place between the hours of 11:00 am and 2:15 pm. There were six (6) classrooms observed with a total of 59 children with 10 staff within the supervision guidelines. The children were observed playing with manipulatives, story time, diaper changing, lunch and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-780-130-A Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required before the child can attend the center.
Evidence:
1.Child #1 started on 9/9/22 and immunization is dated 9/30/22.Plan of Correction: I will ensure we get the immunization record on or before the first day of enrollment.
Standard #: 8VAC20-780-160-A Description: Based on record review, the documentation of a negative tuberculosis (TB) screening was not completed within the last 30 calendar days of the date of employment.
Evidence: Staff #2 (DOH 1/24/22) TB screening was conducted on 5/25/2021.Plan of Correction: I will ensure we have the TB screening results within the 30 day timeframe.
Standard #: 8VAC20-780-280-B Description: Based on observation, potentially poisonous materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. The Preschool 3 class (3 years) has an unlocked cabinet under the changing table with cleaning products and a bottle of bleach solution inside the cabinet.Plan of Correction: A new lock will be ordered but, in the meantime, another locking device was placed on the cabinet.
Violation was corrected during the inspection.
Standard #: 8VAC20-780-500-A Description: Based on observation, the staff did not wash their hands with soap and running water before and after a diaper change.
Evidence: Staff #4 (Infant classroom) did not wash their hands before or after changing a child?s diaper.Plan of Correction: We are doing a retrain with the infant and toddler staff next week.
Standard #: 8VAC20-780-500-B Description: Based on observation, the staff did not clean the diapering surface with soap and water after each use.
Evidence: 1. Staff #4 (Infant classroom) did not use soap and water to clean the diapering surface after each use.Plan of Correction: We are retraining the staff next week.
Standard #: 8VAC20-780-560-J Description: Based on observation, the tables were not sanitized before feeding.
Evidence: The Toddler Staff did not sanitize the tables before children were fed their lunch.Plan of Correction: We are retraining the staff next week.
