Comments:
A focused monitoring inspection was initiated on January 4, 2023 and concluded on January 5, 2023.
There were 47 children present, ranging in ages from infancy to 4 years, with 14 staff supervising. The
inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure all areas and equipment of the center, inside and outside are maintained in a clean, safe, and operable condition.

Evidence: The following observations were made during the inspection:
1. In the Jelly fish classroom, the diapering area had peeling paint throughout.
2. In the Sea Turtles classroom, the was multiple areas of peeling paint on the wall and window sills accessible to children in care.
3. In the Seahorse's classroom, the drawer on the diapering area was missing a knob and there was a visible protruding screw.
4. The Toddler playground slide was observed with a broken plastic part a the top which was accessible to children.
5. Photographs of all the areas identified were taken by the inspector.

Plan of Correction: The center's director notified the owner to request maintenance of all areas identified in the classrooms and the painting and repairs for all areas will be completed over the weekend.

Standard #: 8VAC20-780-280-B
Description: Based on observations, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Guppies and Sea Turtles classroom, there were multiple diapering creams with the labels "keep out of reach of children" and "warning" observed unlocked in each child's individual cubby area.

Plan of Correction: The center's director spoke with teachers in each group and all diapering creams will be stored in a lock cabinet.
