An unannounced on-site monitoring inspection was conducted on 12/27/2022. There were 59 children, ages 8 months-12 years, and 10 staff members present during the inspection. The inspector observed the following: nap, and eating snack. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: Portal for CPS backgrounds and orientation form found on the DOE website.

Standard #: 8VAC20-780-90--A Description: Based on review of five children?s records the center failed to ensure that all children enrolled had the three parental agreements signed by the parent.

Standard #: 8VAC20-780-350-C Description: Based on observation, the center failed to ensure that the ratio was meet when children are in ongoing mixed age group, the staff-to children ratio and group size is applicable to the youngest child in the group.
1.At arrival to the center the school age classroom had 27 children present with one staff person. The youngest child in the grouping was 3 years old. Approximately 10 minutes after arrival two children were picked up making the total number of children 25 with one staff person.
