Comments:
A monitoring inspection was initiated on 11/17/2022 and completed on 01/04/2023 concerning a self-reported incident relating to supervision of a child. Staff records and the child's record were reviewed. Staff interviews were conducted.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person or person(s) responsible for implementation.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on interviews, the center failed to ensure when staff are supervising children that staff shall always ensure their care, protection, and guidance.
Evidence:
On November 10, 2022, staff #1, #2 and #3 lined up 14 children ages 3 and 4 yrs. old at approximately 10:23 am to go to lunch which is located in the public school building. The classroom is in a mobile unit on the school grounds approximately 250 ft. away. After eating lunch and cleaning up, at approximately 10:50 a.m, staff #1 counted 13 children. Staff #2 ran to the classroom where a child, four years old, was found crying and sitting in a chair inside the classroom.
Interviews with staff #1 and #2 determined that the child was left alone and unsupervised in the classroom for approximately 27 minutes.

Plan of Correction: All education staff were provided additional training on active supervision on November 22nd. Training included revising and updating active supervision protocols, counting procedures, and utilizing checklists (rosters) to ensure accountability for all children in attendance.

Description: Based on interviews, the center failed to ensure that children under 10 years of age are always within actual sight and sound supervision.
Evidence:
Interviews with staff #1 and staff #2 determined that a child, four years old, was out of sight and sound supervision for approximately 27 minutes. The child was alone and unsupervised in the classroom when three staff and 13 other children left the classroom to go to the cafeteria inside the school building.

Plan of Correction: Another alternative to traveling outside of the classroom for lunch was implemented and staff and children now eat lunch inside the classroom therefore, reducing the walking transition to the elementary school during the day. Staff were provided additional training on active supervision and individualized social / emotional supports for children. Disciplinary action of staff was also part of the corrective action plan.
