Comments:
An investigation was conducted on 12/15/2022 in response to a self-reported incident received by our office on 12/2/2022, regarding supervision of children. Document review, staff interviews, and review of video footage of the incident, determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-40-E
Description: Based on staff interviews and document review, it was determined that the center did not ensure they followed their own policy regarding child supervision.

Evidence:
A review of the center's policies and procedures, under the area of "Maintaining Supervision of Children" states "Under no circumstances should any child be unsupervised or left unattended." The center staff did not follow the center's own policy when Child #1 was left alone and was not supervised by center employees for approximately 9 minutes from 10:21 a.m. until 10:30 a.m. on December 2, 2012. The time frame was verified by a time stamp on the center's video recording.

Plan of Correction: Created a transition policy for the staff handbook as well as our orientation packet.

Standard #: 8VAC20-780-340-F
Description: Based on video recording review and staff interviews, it was determined that the licensee did not ensure that children under 10 years of age shall always be within actual sight and sound supervision of staff.

Evidence:
1. A review of the center's video recording and interviews with center staff confirmed that Child #1 (four years old) was not within actual sight and sound supervision of a staff member for approximately 9 minutes.
2. On December 2, 2022, there were 15 children attending the PreK classroom and 3 staff members supervising them.
3. At approximately 10:20 a.m., 14 children were observed in the PreK classroom lining up to go to the Sanctuary.
4. At 10:21 a.m. Staff #1, #2 and #3 exit the classroom with 14 children without conducting a head count.
5. At approximately 10:22 a.m. Child #1 was observed walking out of the bathroom into the empty classroom.
6. At 10:30 a.m. a staff member is observed entering the PreK classroom, after the center received a phone call from a parent to let administration know that they had observed, through the center's video camera system, that a child was alone in the PreK classroom.
7.The video recording confirmed that Child #1 was left alone and unsupervised in the PreK classroom on December 2, 2022, from 10:21 a.m. to 10:30 a.m.

Plan of Correction: Two of the three teachers were written up and filed. The third teacher was not because it was her first day.
