Comments:
A renewal inspection was completed on 01/05/202. There were 48 children present with 9 staff supervising. Tow cooks and the family advocate were also present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, five staff records, and board officer records were reviewed. The inspection started at 9:30am until 11:30 at the main office to review records and continued at the center from 11:30am -12:50.

The new application, the provider portal, and background checks were discussed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-40-D-1-a
Description: Based on record review and interview, the center failed to have a sworn statement or affirmation, search of central registry and criminal history record check whenever an applicant, licensee, whenever an applicant, licensee, approved family day home operator, or registrant changes before the end of 30 days after the change.
Evidence:
1. There was no sworn statement, central registry and criminal history record check for board officer #4 on record. 2. The executive director verified the records had not been completed.

Plan of Correction: The executive director will make sure the officer will get all the required background checks required.
