A complaint investigation was initiated on December 16, 2022, and ended on January 17, 2023, regarding children in the 12 month old classroom given choking items for activities and parents having entry in the classrooms. Interviews were conducted and documentation were reviewed. There was not a preponderance of evidence to support the allegation entry in the classrooms. The complaint was deemed valid with regard to unsafe items being accessible to children for activities. The complaint related violations and one non-related violation are identified in the Violation Notice.

Standard #: 8VAC20-780-280-I Complaint related: No Description: Based on classroom documents and observations, the center used hazardous art and craft materials with children.
Evidence:
1. In review of documentation, in the wobbler classroom, staff did a water bead activity with children. A child was sitting in high chair with water beads. The child was touching waterbeds with finger. The water beads is not an appropriate activity due to the size of the item which can be a choking hazard.
2. During walk-through of the toddler classroom, cotton balls were on a tray on a bookshelf. Staff stated cotton balls were used for an stretch activity with the children.
3. The toddler classroom did not have a choking tube in the classroom which is used to determine if the item for activity is a chocking hazard.Plan of Correction: Not available online. Contact Inspector for more information.
