A renewal inspection was initiated on December 30, 2022 and concluded on January 19, 2023. There were 62 children present directly supervised by 16 staff. The children were observed in center play, transitioning from outdoor play and individual activity.
Limited monitoring of child care requirements occurred during today?s visit. Licensing inspector monitored supervision of children, physical plant, staff/child ratios, adequate and approved space, programming, and nutrition. Licensing Inspector reviewed 8 records and 5 children records. No medications on-site for review.

Standard #: 8VAC20-780-70 Description: Based on observation, interview, and staff record review it was determined the center did not maintain and make accessible records and reports on staff.
Evidence:
1. The center failed to make accessible the required documents to monitor and complete the required inspection within the required time frame.
2. The inspector provided the center with the monitoring inspection request on 12/30/2022.
3. Inspector spoke with Director on January 3, 2023, regarding records that were not accessible during the on-site inspection.

Standard #: 8VAC20-780-270-A Description: Based on walk-through of the classrooms, the center did not maintain areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the Jellyfish and Octopus classroom, bathroom toilets had protruding screws on the base toilets. The toilets were missing caps to cover the protruding screws.
2. In the Angelfish classroom, a floor mat had torn material that was accessible to children.
3. In the Goldfish classroom, 6 cubes had torn material in the corners that was accessible to children.
4. In the Seahorse classroom, two red couches had tears in the arm and seat that was accessible to children.Plan of Correction: Not available online. Contact Inspector for more information.
