Comments:
An unannounced monitoring inspection was conducted today, January 4, 2023, to determine the centers compliance with licensing standards. The licensing inspector was on site from approximately 3:30 pm until 6:00 pm. The Director was present and assisted with the inspection. There were 33 children present, ranging in ages from ten months to five years, with six staff supervising. Children were observed playing with blocks, coloring, baby dolls and dancing. Infants were playing and interacting with staff on the floor mats. Staff interacted with children in a positive manner. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming,
medication, special care and emergencies and nutrition. A total of four child records and five staff records were reviewed. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have uestions regarding this inspection, please contact the licensing inspector.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of five staff records, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children for one staff member.

Evidence:

The record of staff #4, employed 08/19/2022, did not contain the results of a tuberculosis screening exceeding the first day of employment.

Plan of Correction: Per the director: I will have the staff member obtain a tuberculosis screening.

Standard #: 8VAC20-780-210-A-4-a
Description: Based on review of five staff records, the center did not ensure one staff, assigned as a program leader, met program leader qualifications.

Evidence:


1. Staff #3, employed 09/21/2022 as a program leader, had documentation 18 hours of training that was completed prior to six months before beginning work when 24 hours is required within six months of beginning work or within 30 days after being promoted or beginning work.
2. The record of #3, identified as a program leader and in charge of a group of children, did not have documentation of having fulfilled a high school program or the equivalent

Plan of Correction: Per the director: I will have staff complete 24 hours of training and provide documentation of their high school diploma to ensure they meet program leader qualifications.

Standard #: 8VAC20-780-240-A
Description: Based on review of five staff records, the center did not ensure one staff completed the Virginia Department of Education-sponsored orientation course within 90 days of employment.

Evidence:

The record of staff #1, employed 01/13/2022, had documentation of completion of the Virginia Department of Education-sponsored training dated 04/18/2022, exceeding 90 days from employment.

Plan of Correction: Per the director: I will ensure staff complete the orientation training within the 90 days from employment.
