Comments:
An unannounced monitoring inspection was conducted on 01/09/2022 from 10:00am to 12:40pm. There were 81 children ages 5 months to age 5 years old supervised by 13 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, 14 medications with corresponding authorization records, emergency drills, injury reports, and emergency supplies were reviewed. Children were observed participating in writing, math activities, art, reading and outside play. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-350-Q
Description: Based on observation and staff interview, the center did not ensure they have a parent?s written permission and a written assessment by the program director and program leader to assign a child to a different age group.

Evidence: Child #1 (DOB 01/27/2021) was assigned to a classroom containing 13 children and 2 teachers. The appropriate supervision ratio for a child of this age is 1 staff member per 5 children. The child?s file did not contain written parent permission to be re-assigned to a different age group.

Plan of Correction: Classroom transfer consent form was obtained by parent and signed by teachers and
administration.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not ensure that proper handwashing procedures were followed.

Evidence: In the Threes classroom, after a staff member assisted with a soiled diaper while toilet training, they moved on to assist the next child who was toilet training and did not wash their hands before assisting the next child.

Plan of Correction: Diaper changing and handwashing protocols were reviewed and re-trained
with all classroom staff.

Standard #: 8VAC20-780-500-B
Description: Based on observation and staff interview, the center did not ensure that disposable diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal. They did not ensure that

Evidence:
1)In the Threes classroom, the foot-operated storage system for disposal of diapers was not able to be operated by foot causing the staff member to touch the exterior surface of the trash can with their hand or a soiled diaper to open the trash can.
2)In the Toddler classroom, the foot-operated storage system for disposal of diapers was stored inside a cabinet causing the staff member to touch the trash can each time a soiled diaper needed to be disposed.

Plan of Correction: Both classroom diaper bins were moved to accessible areas for foot pedal operations.
Both bins were tested and classroom teachers retrained in diapering policies.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center did not ensure that diaper cream was kept inaccessible to children.

Evidence: In the Threes classroom, there was a diaper cream stored in an unlocked drawer on the diaper station where children have access.

Plan of Correction: Diaper cream was removed and moved to locked cabinet.
