Comments:
A self reported incident was received on 10/31/2022. A monitoring inspection was conducted on 11/16/2022 and concluded with a local agency representative on 12/22/2022. The inspector reviewed compliance in the area of discipline and reporting. Staff interviews were conducted and documentation was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-410-1
Description: Based on staff interviews, the facility failed to ensure forbidden actions such as rough handling or striking a child were not used. Evidence: On 10/28/2022, Staff A observed Staff B rough handle and strike Child A (Age 2). Staff B was observed taking Child A by the arm, forcing the child to sit on the floor against the wall, pushing his head forward, and smacking him on the back of the neck at least three times.

Plan of Correction: Staff B resigned; the center contacted CPS and self reported to licensing.
The care, protection, and safety of the children in our care is our top priority. Once the incident was reported to the director, we immediately started the investigation and placed the teacher on unpaid administrative leave; the employee is no longer employed. We then contacted the appropriate agencies ourselves to report the incident. The staff member involved described it as tapping on his head to get his attention. The current staff who care for the children received positive guidance training regarding developmentally appropriate interactions with children. The family involved in the incident understands that our number one priority is the safety of our children, and they are still enrolled in our care.
