Technical Assistance:
Discussed monitoring of medication forms and insect/pest control with the Director.

Comments:
Today an unannounced renewal inspection took place between the hours of 11:20am and 1:10pm. There was a total of 63 children with 13 staff within the supervision guidelines. The children were observed having circle time, eating lunch and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency materials were observed during this inspection. Violations:
Standard #: 8VAC20-780-70
Description: Based on a review of staff records and staff interview, all required documents were not kept for each staff person.

Evidence:
1. Staff #2 (doh: 1/11/2023) did not have documentation of 2 out of 2 required references prior to employment.
2. The Director stated that they have not contacted any references yet.

Plan of Correction: We will get the references for the staff member and will get them prior to employment moving forward.

Standard #: 8VAC20-780-245-A
Description: Based on a review of staff records, all staff did not complete the annual minimum of 16 hours of training appropriate to the age of children in care.

Evidence:
1. Staff #3 (doh: 8/11/2020) was only able to provide documentation of 8 total hours of training appropriate to the age of children in care for the preceding year.

Plan of Correction: We will make sure we come up with a plan to keep track of the training hours, have certificates, and make sure everyone gets 16 hours per year.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Toddler 1 classroom, there was an unlocked bathroom/closet that had multiple cleaning materials on the floor.
2. In the Toddler 2 classroom, there was an unlocked bathroom/closet that had multiple cleaning materials on the floor.

Plan of Correction: We will relocate the cleaning supplies in both classrooms to a locked closet today.

Description: Based on observation, a nonabsorbent surface for diapering or changing was not always being used for children under 3 years of age.

Evidence:
1. In the Toddler 2 classroom, there is not a nonabsorbent surface for diapering or changing. The material being used is not a changing pad and does not appear to be nonabsorbent.

Plan of Correction: We will order a changing pad.

Standard #: 8VAC20-780-500-B
Description: Based on observation, disposable diapers were not disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff
member's hand nor the soiled diaper touches an exterior surface of the storage
system during disposal.

Evidence:
1. The only available diaper storage system in the infant room was not foot-operated and the staff would have to touch an exterior surface when disposing diapers.

Plan of Correction: We will order a step trash can today.

Standard #: 8VAC20-780-550-F
Description: Based on record review and staff interview, lockdown procedures were not practiced at least annually.

Evidence:
1. There was no documentation of a lockdown drill for 2022 at the time of the inspection.
2. The Site Director stated that a lockdown drill was not completed during 2022.

Plan of Correction: We will make sure we do one lockdown drill per year and document it.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared infant formula was not dated and labeled with the child's name.

Evidence:
1. In the Toddler 2 classroom, there was one bottle found with no name and no date and one bottle found with a name, but no date.

Plan of Correction: We will make sure all bottles are labeled with name and date moving forward.
