Comments:
A monitoring inspection was initiated on January 18, 2023 and concluded on January 18, 2023. There were 71 children present, ranging in ages from 10 months to age 11, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 7 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and provider interview, the licensee did not ensure, all staff had obtained updated background checks as required every 5 years.
Evidence: Staff records were reviewed and 1 out of 7 did not have an updated Sworn Statement. The most recent Sworn Statement for Staff #5 was dated 12-19-2017. Staff #5 was observed working during the inspection.

Plan of Correction: Staff has completely filled out the sworn statement disclosure statement

Standard #: 8VAC20-780-140-A
Description: Based on record review and provider interview, the licensee did not ensure to obtain documentation that each child has received a physical examination by or under the direction of a physician, before the child's first day of attendance; or within 30 days after the first day of attendance.
Evidence: The record for child #1, (first date of attendance 11-8-2022) did not include documentation of a physical.

Plan of Correction: Parent has been notified & will bring in completed up to date physical documentation.

Standard #: 8VAC20-780-150-B
Description: Based on record review and provider interview, the licensee did not ensure the physical examination included the signature of a physician, his designee, or an official of a local health department

Evidence: The record for child #6, (first date of attendance 8-1-2022) did not include a physicians or designated individuals signature.

Plan of Correction: Parent ahs been notified of a physical need that is filled out with physicians information, signature noted.

Standard #: 8VAC20-780-60-A
Description: Based on record review and provider interview, the provider did not ensue to maintain an up-to-date record for each enrolled child with all the required information.
Evidence: The provider confirmed the following items were missing from 3 out of 7 of the children?s records reviewed.
1. The records for child #2 and child #4 did not include the address, telephone # and place of employment for one of the parents listed or the employment phone # for the other parent listed
2. The record for child 7 ? did not include the employment phone # for one of the parents listed

Plan of Correction: Parent was notified of information needed to complete their child's file with incomplete /missing information.
