Comments:
An unannounced 60-day monitoring inspection was conducted on Wednesday, January 18, 2023 to determine the centers compliance with licensing standards. The inspection begin at approximately 9:15 AM and concluded at 11:30 AM. Today?s census included 139 children with 20 staff supervising. The inspector observed children eating a breakfast, playing with age-appropriate materials, completing a worksheet, and listening to a story. The inspector observed all 10 classrooms, playgrounds, buses, and restrooms. Staff was observed interacting with children in a positive manner and the inspector interviewed staff and children when time allowed. The classrooms are equipped with age and stage-appropriate materials. The center's first aid kit and nonmedical emergency supplies were inspected and found complete. The required postings were inspected and found to be in compliance. Liability insurance, health inspection, and drills were all in compliance. There were 10 children's and 2 staff records reviewed. Medication was reviewed for 6 children and found to be complete. There were violations cited during this inspection. Description: Based on observation, areas and furnishings of the family day home were found in an unsafe condition.
Evidence:
1. A blue child size chair in the reading area located in the PreK classroom, was torn on both arms of the chair exposing the webbing.
2. A child size red chair in the Early Preschool classroom, was torn on the corners of the arm exposing the webbing.

Description: Based on observation, and interview, the center did not ensure when food was provided the most recent USDA nutritional requirements.
Evidence:
1.The inspector observed children eating breakfast that did not contain all of the USDA nutritional requirements.
2. Breakfast consisted of orange slices, strudels, and water.
3. Milk was not provided for breakfast.
