Technical Assistance:

Discussion of upcoming expiration dates (5 years from the date of finding) for many of the staff background checks that are on file at the Child Day Center. Discussion of training (both the 16 hours of annual training, and the 10 hour VA Pre-service training required within 90 day of each new staff person's start date.

Comments:

This monitoring inspection was initiated by licensing staff and concluded on 01/19/2023, between the hours of 9:45 am to 1:45 pm. There were 117 children present, ranging from infant to school-age, with 16 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 6 children records, 3 administrative files, and 5 staff records were reviewed. Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center. Standard #:	8VAC20-780-160-C
Description:	Based on review, 1 of 3 Administrative Files indicated that it has been more than two years from the date of the tuberculosis documentation (TB) on file at the Child Day Center. Evidence - 1. On the date of inspection (01/19/2023), current TB documentation was not available for review for Admin file #1. 2. This same documentation was cited during a previous inspection conducted on 07/06/2022.
Plan of Correction:	An appointment was scheduled for next week in order to get updated TB documentation to be placed on file.
Standard #:	8VAC20-780-270-A
Description:	Based on observation, a few areas inside of the center were not maintained in a safe and/or operable condition. Evidence - 1. On the date of inspection (01/19/2023), a blind cord observed in the early Montessori room was broken. The cord hung low enough to where is was accessible to the children in care. 2. There were two holes observed in walls within the center. One, was located in the multi-purpose room behind one of the doors. The second, was located within Room #2 above a sink attached to the wall. It appears that the hole was caused when the soap dispenser (that was previously installed) was removed.
