Comments:
A renewal inspection was conducted on 01/12/2023 from approximately 10:45 a.m. to 1:55 p.m. There were 25 children present ranging from 2 years old to 5 years old and 3 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child records and five staff records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 01/27/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on child record review, the provider failed to ensure that one of five records
contained all the required information.
Evidence:
The record for C1, enrolled 09/6/20220 failed to contain documentation that the child received immunizations required by the State Board of Health before the child attended the center. The documentation in the record noted that the child was ?not age appropriately immunized.?

Description: Based on observation, the provider failed to maintain the outside play areas in a safe and
operable condition.
Evidence:
On 01/12/2023, the inspector observed: the outdoor porch benches used by school age children with deteriorating wood seat boards and exposed nails that could entangle clothing or snag skin; deteriorating boards on the stair treads between the porch and the play area with exposed nails; and a window located in the pre-school play area and within reach of children had deteriorating wood and expose metal flashing that could entangle clothing or snag skin.
