Comments:
The Licensing Inspector initiated and completed an unannounced monitoring inspection on 1/18/2023. The inspector was on site from 10:35am until 1:30pm.

There were 37 children under the care of 8 staff. The ages of the children ranged from seven months to five years old. The children were observed during indoor free play and mealtime. Four children?s record and four staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, the center failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.

Evidence: Child #2, which was enrolled on 08/29/2022 had no documentation of a physical on file during the inspection on 1/18/2023

Plan of Correction: Director will reach out to mom on 1/18/23 about the form. She has until 1/27/23 to return it.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to maintain the playground in a clean, safe, and operable manner.

Evidence: On the door of the wooden gate, the slacks of wood and been broken creating sharp, pointed edges which create a snagging hazard for children.

There was a screen from a window on the ground on the blacktop. The screws that hold the screen in place were protruding upward causing an impalement hazard for children. No children were observed on the playground

Plan of Correction: Fence will be repaired by 1/27/2023. Screen was removed from playground on 1/18/2023.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure that all climbable playground equipment
had the required resilient surfacing.


Evidence: Observed non-measurable amount of resilient surfacing under the required fall
zone. The required amount is 6 inches of resilient surfacing,

Plan of Correction: Mulch will be added to the playground by 1/27/23.

Standard #: 8VAC20-780-530-A-1
Description: Based on record review, the center failed to maintain a current CPR certificate for each staff member within the first 90 days.

Evidence: Staff #1, which was hired on 10/11/2022 and Staff #4, which was hired on 08/31/2022, had no record of an CPR certificate during the inspection on 1/18/2023

Plan of Correction: Class for certification is scheduled on 1/25/23.

Standard #: 8VAC20-780-530-A-2
Description: Based on record review, the center failed to maintain a current First Aid certificate for each staff member within the first 90 days.

Evidence: Staff #1, which was hired on 10/11/2022 and Staff #4, which was hired on 8/31/2022, had no record of a First Aid certificate during the inspection on 1/18/2023.

Plan of Correction: Class for certification is scheduled on 1/25.
