A monitoring inspection was conducted on 01/23/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The inspector emailed the director/provider a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
1. The refrigerator in the infant room had melted ice pooling inside of it and the thermometer read 46 degrees Fahrenheit. Staff stated the refrigerator has been broken since the summer.
2.There was peeling paint throughout the building; The trim on the walls throughout the classrooms were chipped and peeling.
3. There were 2 bicycles in the gymnasium that each had a handle grip missing, exposing the metal end.
4. On the plastic coverings for a crib mattress was torn and the foam was exposed in two areas.

Plan of Correction: Plan of Correction: Will be corrected 3/15/2023. Moving forward safety captain and staff will inspect our physical environment meets safety standards daily. Any deficiencies will be reported to management within 24 hours. Management will also inspect through walk throughs to ensure compliance.

Standard #: 8VAC20-780-280-G
Description: Based on observation the center did not ensure hazardous substances were clearly labeled when kept in substitute containers.

In the Infant room there was a spray bottle on a shelf containing a liquid substance. The bottle was not labeled indicating the contents of the bottle.

Plan of Correction: Plan of Correction: This will be corrected by 02/17/2023. Staff will be retrained on all hazardous substances and chemical procedures. All staff are trained during orientation on safety procedures regarding hazardous substances and chemicals. Staff will be doing weekly compliance safety checks. The safety captain will be doing daily walk throughs. Management will do periodic inspections to make sure policies are being upheld.

Standard #: 8VAC20-780-340-A
Description: Based on observation, interviews and a record review the center did not ensue when staff are supervising children, they shall always ensure their care, protection, and guidance.

Observations were made in the toddler and two year-old room from around 11:45am -12:30pm as the class was preparing for nap.

1. Staff A , designated program lead in the 2 year -old classroom, did not provide guidance or care to children as she attended to classroom chores and did not intervene as children were observed rolling on the carpet, climbing the bookshelf, climbing the sand table and a stack of 11 cots, eating the corners of books, jumping on cot sheets, crying and wandering the room.

2. In the twos room the backdoor leading to the outside playground was not locked. The District Manager confirmed the door remains unlocked per Fire Marshall instructions. None of the staff in the twos room acknowledged Child #1, who attempted twice to open the back door that leads to the playground.

3. In the twos room, Staff A did not provide care, protection or guidance when she noticed two children were eating peas off the floor that had not been cleaned up from lunch. Staff A stated, ?I swept so you couldn?t eat off the floor.? The two children continued eating off the floor as Staff A walked away with no redirection.

Plan of Correction: Plan of correction: Center management has a well-defined action plan to ensure the safety, guidance, and supervision of the children. This plan includes, but is not limited to: Supervision, positive guidance and redirection, and meal time expectation training. This training will be completed by 3/17/23. Training and rosters will be submitted to the office by 3/21/2023. Observations and coaching will occur weekly and utilized to determine additional steps. Additional classroom resources and room arrangement will help support the teachers to ensure a safe learning environment and smooth transitions.

Standard #: 8VAC20-780-370-4
Description: Based on observation and interview, the center did not provide infants with comfort as needed.

1. In the infant room Staff did not offer comfort to a crying infant. child #1 was observed on her back crying on a mat on the floor, her face visibly red. When the inspector asked about the crying child, staff replied that she is just getting back from her grandparent's house and was used to being held. Staff stated, "we can't hold her all the time. We have to feed the other children. "We don't have time for it."

2. In the twos room staff did not provide comfort to Child #5 when needed.
Child #5 was observed crying and wandering the classroom holding a blanket. Child #5 repeated, ?hurry up,? to a member of management looking for his cot to lay down on for nap. Staff did not comfort Child #5 and continued preparing cots for other children, as his was the 2nd to last to prepared and put down for use.

3. Staff in the twos room did not provide comfort to Child #7 who was observed on her cot crying. When asked why the child was upset, staff stated she was new as the child continued to cry.

4. Staff in the twos room did not provide Child #6 with comfort as needed. Child #6 was observed wandering the room and sitting and/or lying on other children?s cots in an attempt to lay down for nap. Staff removed him each time, however, did comfort or talk to the child. His cot was the last to be put down.

Plan of Correction: Plan of correction: Teachers have been instructed to reach out to the management team for additional support when they are attending to the needs of other children and are unable to quickly respond to another child?s needs. Teachers must respond to a child?s needs within a reasonable amount of time.
In Service Day; 2/20/23 ? Staff will attend two trainings that directly relate to meeting the social emotional needs of the children: The Power of Emotionally Supportive Relationships and Impacting Children?s Learning Though Positive Interactions. The training roster and agenda will be submitted to the office by 2/25/2023.

Standard #: 8VAC20-780-440-H
Description: Based on observation and interview the center did note ensure all crib requirements were met.
1. In the infant 2 room there were 8 cribs that did not have at least 12 inches of space between the sides and ends of the cribs, nor was there 30 inches of space between service sides and other furniture where that space is the walkway for staff to gain access to any occupied crib.
2. Infant staff stated when cribs are occupied, they are not moved and babies are placed in the cribs with the current spacing.

Plan of Correction: Plan of Correction: The room arrangement was corrected the week of 02/06/2023. Staff will be retrained on safe sleep and classroom set-up environment by 02/17/2023. Staff will ensure all furniture is put back daily to ensure safe set up. Safety captain will do daily inspections. Management are conducting random classroom observations weekly to ensure compliance.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering.
1. In the 3 year-old room there were floor chairs stored on top of the surface of the diaper changing table..
2. In the infant room, the changing table's sink was filled with empty bottles, cups and lids. Staff stated they use the changing table sink to wash the baby's bottles.

Plan of Correction: Plan of Correction: Was corrected on 01/30/2023. Moving forward staff will ensure that no items are stored on the changing tables. Management will conduct daily audits to ensure we are meeting diaper requirements.

Standard #: 8VAC20-780-570-E
Description: Based on observation the center did not ensure all special feeding requirements were met.
In the infant room there were 5 bottles of infant formula that were not labeled with the name of the child it belonged to or the date. The lead teacher in the classroom stated they don't have time to label the bottles.

Plan of Correction: Plan of Correction: Staff will be retrained by 02/17/2023 on infant feeding requirements. Moving forward management will conduct daily audits of infant bottles to ensure they are properly labeled. Labels have also been made available outside the classroom for parents to take on an as needed basis.
