Technical Assistance:
The designated emergency contacts should be individuals different from the parent.

The menu should list all foods served on each day.

Comments:
An unannounced renewal inspection was conducted on 01/24/2023 from 4:25 pm - 6:00 pm. At the time of entrance there were 37 school age children in care with four staff. Children were observed during outdoor play and playing games in the cafeteria.
Records were reviewed for five children in care. The inspector additionally reviewed information for a serious injury that occurred on 01/10/2023.
Five staff records were reviewed on 01/25/2023 at the Greenbrier North YMCA where staff files are maintained.

Information gathered during the inspection determined non-compliances with applicable standards and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to ensure that documentation of the TB screening shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
The TB screening for staff 1 was completed on 01/04/2023 which is after her hire date of 09/14/2022.

Plan of Correction: Administrative staff will ensure that all new staff have a TB screening prior to employment which will have been completed within 30 days.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. The enrollment record for child 1 lacked the following required information:
a. A work phone number for one parent;
b. The name address and phone number of a second emergency contact.
2. The enrollment record for child 2 lacked the following required information:
a. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached;
b. The work phone number and place of employment of each parent who has custody;
c. The home address of each parent who has custody.

Plan of Correction: Administrative staff stated the each parent will be asked to provide updated/current information.

Standard #: 8VAC20-780-240-A
Description: Based on record review, the center failed to ensure that staff complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. Staff 1, hire date 09/14/2022, has not completed the Virginia Department of Education training course.

Plan of Correction: Administrative staff stated that staff 1 is in the process of completing this training.
