Technical Assistance:
The center administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container.
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Comments:
A monitoring inspection was initiated on 1/18/23 and concluded on 1/18/23. There were 65 children present with 11 staff, ranging in ages from infants to five years. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, the center failed to ensure staff provided documentation of a negative tuberculosis (TB) screening completed within the last 30 calendar days of the date of employment.

Evidence:

1. Staff 3's employment start date is documented as 12/1/22. The TB screening document is dated 4/28/22.
2. The director verified the date of employment and stated they do not have another TB for staff 3.

Plan of Correction: Staff 3 will be required to update the TB screening. In the future the director will ensure new staff provide a TB screening that has been completed within the last 30 calendar days of the date of employment.

Standard #: 8VAC20-780-510-F
Description: Based on observation and interview, the center failed to obtain medication authorization from the parent and physician for the center to administer medication for two children in care.

Evidence:

1. The center has an EpiPen and Benadryl for child 5. The center has a care plan on how to administer the medication but does not have permission to administer the medication from the parent and the physician.
2. The director verified the center does not have medication authorization for the EpiPen and Benadryl for child 5.
3. The center has an Inhaler for child 6. The center has a care plan on how to administer the medication but does not have permission to administer the medication from the parent and the physician.
4. The director verified the center does not have medication authorization for the Inhaler for child 6.

Plan of Correction: The parents of child 5 and child 6 have been informed sign medication authorization forms and to have the physician sign the medication authorization forms. In the future all long term medications will be required to have parent and physician medication authorization forms.
