An unannounced monitoring inspection as conducted on January 26, 2023 from 12:10 p.m. to 1:10 p.m. Regarding a self-report received on January 24, 2023. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Description: Based on observation, documentation and interview, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
1. During an interview on January 26, 2023, Staff #2 revealed that Child #1 fell from a swing January 20, 2023 resulting in a broken collar bone.
2. Staff #2 stated that Child #1, age 2 years, was lifted onto the swing because the swing was too high for the child to get onto by themselves.
3. Staff #2 stated that Child #1 let go of the swing, fell forward from the swing, landed on their hands on the ground, with their arms straight, which resulted in a broken collar bone.
4. The distance from the bottom of the swing to the ground measured 33?. The swing was observed to be located on a hill and attached to a tree branch.Plan of Correction: Not available online. Contact Inspector for more information.
