Technical Assistance:
Discussed the systemic/repeated non-compliance found during the last two inspections.

Discussed the Phase II training as a resource to assist with understanding the application and intent of the Standards.

Discussed the maintenance and cleanliness of the building and the need to complete random observations of each classroom to ensure maintenance.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 25 children with 7 staff were onsite today. Observation of all 4 occupied classrooms was made along with the playground areas. The children were observed transitioning to nap time and finishing lunch. A total of 3 children's records and 2 staff records were rechecked from the December inspection along with 1 new staff hire were reviewed. The violations found during the December inspection were reviewed and several repeated violations were found today. A corrective action for each was discussed with the Head of School.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation and staff statements, the center failed to maintain a current written list of children's allergies.
Evidence:
1. Staff #1 stated that child #1 in the Nido 1 classroom was recently diagnosed with a food allergy. The written list of children's allergies was not updated to include the child's food allergy.
2. The written list of children's allergies included one child who no longer is enrolled.
3. The written list of children's allergies was not updated to reflect the correct classroom for Child #2.

Plan of Correction: The list of food allergies will be updated and routinely monitored to ensure they remain current.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the Toddler classroom, two areas of the building was not maintad in an operable condition and/or safe condition.
Evidence:
1. The bracket to hold toilet paper on the right side of the bathroom was loose and missing the piece of equipment to hold the toilet paper in place.
2. The window ledges in the Toddler classroom has paint that is peeling and accessible to the children in care.

Plan of Correction: A work order request will be placed to have both these areas repaired.

Standard #: 8VAC20-780-510-I
Description: Based on review of medication, one medication was not properly labeled.
Evidence:
An emergency medication found in the Children's House classroom did not have a pharmacy prescription label.

Plan of Correction: The medication was stored within the pharmacy labeled manufacturer's packaging which was located on the first level of the center.

Description: Based on observation of the Children's House classroom, medication was found unlocked.
Evidence:
The medication box was found unlocked inside an unlocked cabinet in the classroom.

Plan of Correction: Medication was locked immediately

Standard #: 8VAC20-780-510-P
Description: Based on review of medication authorizations, one authorization expired and the medication was either not picked up by the parent or disposed of.
Evidence:
An authorization for an antihistamine expired on 12/22/2022. The authorization was not renewed, the medication was not picked up by the parent or disposed of within 14 days of the expiration of the authorization.

Plan of Correction: Staff will discuss with the parent if a new authorization is requested and if not, the medication will be returned to the parent.

Standard #: 8VAC20-780-570-B
Description: Based on observation of the Nido 2 classroom, a bottle was used while one child was in a sleeping location.
Evidence:
A 9 month old infant was observed drinking a bottle while laying down on a boppy pillow.

Plan of Correction: All children who can hold their own bottle will be seated in a high chair or feeding table while drinking.
