Comments:
An unannounced renewal inspection was conducted onsite. There were 14 children in care ages five months through four years old with four staff. Children and staff records were reviewed. A walking tour of the child care center inside and outside was completed. Children were observed in teacher and self-directed activities including floor play, lunch and nap. Additionally during the inspection first aid kit and emergency supplies were reviewed as well as health and fire inspections, emergency evacuation drills and required postings. Violations observed include physical plant, ratio, naptime supervision and unlocked chemicals. These violations were reviewed during a telephone exit interview on January 26, 2023 and appear on the violation notice.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to maintain areas in safe and operable condition.
Evidence:
1 - There was a large bag of toys, a ladder and a ride on toy leaning up against the gas water heater in the laundry room. These items pose a fire hazard.
2 - There was a plastic bag filled with recycled bags within reach of children due to an open safety gate. The accessible plastic bags pose a suffocation hazard.
3 - There are large plastic bags/wrapping accessible in the bathroom on the first floor.
4 - The wooden banister on the staircase leading to the second floor had separated and exposed splitting wood and a nail. The separation also creates a pitch point.
Outside
5 - There was a deteriorating cardboard box filled with sand, within reach of children.
6 - There was a turtle shaped sandbox filled with rain water with in reach of children.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method.
Evidence:
1 - The closet in the infant care room bathroom which contains a variety of cleaning and sanitizing chemicals was partially open and unlocked making the chemicals inside accessible to children in care.
2 - In the preschool classroom on the second floor, in an unlocked closet there was a spray bottle labeled bleach water.
3 - In the bathroom on the first floor there was a solid air freshener on the back of the toilet, within reach of children. The label states keep out of reach of children.

Description: Based on observation and interview the center failed to maintain required ratio of one staff to four children in classrooms serving children birth to 16 months old.
Evidence:
1 - Upon entering the building there were five infants in the infant care room with one staff.
2 - Staff #1 stated that Staff #2 had left the child care room at 11:55am.
a. Staff #2 returned to the classroom at 12:08pm
3 - Staff #2 was observed in the kitchen area of the building preparing lunches for children in the toddler and preschool classrooms. The door to the infant classroom was closed. The kitchen area is not a part of infant care room.

Standard #: 8VAC20-780-350-M
Description: Based on observation and interview the center failed to demonstrate compliance with one staff member not supervising more than one room or area during rest time.
Evidence:
1 - Staff #4 was observed sitting between two classrooms, the toddler/two's room and the three/four year old room during nap/rest time.
2 - Staff #4 stated that she was supervising both rooms as another staff had gone to lunch.
