A monitoring inspection was conducted Wednesday, January 25, 2023. The licensing inspectors were onsite, completing the inspection from approximately 9:40am until 1:15pm. There were 38 children present, ranging in ages from 13 months to 5 years, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff members? records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-130-A Description: Based on review of five children?s records, the center did not obtain documentation of required immunizations prior to one child attending the center.
Evidence:

Standard #: 8VAC20-780-140-A Description: Based on review of five children?s records, the center did not obtain documentation of the physical examination by or under the direction of a physician within the required timeframe.

Standard #: 8VAC20-780-280-B Description: Based on observations, the center did not ensure all hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. Two containers of cleaning materials were stored in an unlocked cabinet in the Pre-K classroom labeled Mystery Box Cabinet. The cleaning materials were labeled with words Warning and Caution ? Keep Out of the Reach of Children. There were 10 children in the Pre-K classroom during the inspection.
2. Three containers of cleaning materials were stored in an unlocked cabinet located in the children?s bathroom. The cleaning materials were labeled with words Warning and Caution ?
