Technical Assistance:
Discussed medication observed in the center that did not have a corresponding authorization form. The center may not administer the medication until a current form is obtained.

Comments:
An unannounced renewal inspection was conducted today from approximately 10:00am-11:20am. There were 38 children directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, napping, drawing, and playing a word game. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Evidence:
1. A fan was observed on a shelf in the toddler room with a dusty cover.
2. A return vent located behind a crib in the Infant A classroom was observed to be dusty.
