Comments:
An unannounced renewal inspection was conducted today from 11:00am-12:15pm. There were 35 children directly supervised by 7 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and getting ready to eat lunch. Hand-washing procedures were also observed. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-160-C
Description: Based on record review, it was determined the center didn't obtain a current TB Screening before two years since the last check.
Evidence:
1. Staff #3's TB Screening document is dated November, 2020.

Plan of Correction: Will check all records to ensure all staff are current and will send staff to get check.

Standard #: 8VAC20-780-50-A
Description: Based on observation, it was determined Staff records were treated confidentially.
Evidence:
1. During the Inspection, staff training files were requested. The Director had to go into the classroom to request the staff's binders. These Binders were not in a locked space.
2. Staff #1 and #3 had confidential information in these binders.

Plan of Correction: Will keep confidential info in the office.

Standard #: 8VAC20-780-550-G
Description: Based on record review of emergency drills, the center does not include complete documentation.
Evidence:
1. The Director keeps documentation of emergency drills on computer and a sample FDH Log.
2. The log does not include who conducted the drill, the method used to notify staff and children of a drill, the number of children and staff that participated, and how long it took.

Plan of Correction: Will ensure all future drills are complete with information required.

Standard #: 8VAC20-780-550-P
Description: Based on record review of injury reports, it was determined that injury reports do not include all required documentation.
Evidence:
1. An injury report was reviewed in which a child fell while returning for a fire drill.
2. The record did not include prevention recurrence of this type of injury and signatures of either two staff or one staff and the parent's signature.

Plan of Correction: Will ensure all future injury reports are complete.
