Comments:
A renewal inspection was conducted on 01/26/2023. There were 32 children present, ranging in ages from 2 to 6 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 6 staff/board member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the Program Director during the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Base upon observation and staff interview, the facility has not ensured that equipment is maintained to be clean.
Evidence:
1. A nap mat in the PK2 room was placed out for a child for nap time. The mat had several long tears making it impossible to appropriately sanitize the mat between uses by the child.
2. Staff 1 verified that the mat was torn.

Plan of Correction: The facility responded with the following:
The nap mat was immediately replaced with a new mat. All mats were checked to ensure they are good condition that would allow for effective sanitizing.

Standard #: 8VAC20-780-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using a safe locking method that prevents access by children.
Evidence:
The teacher's desk drawers in the PK4 classroom were unlocked. In the drawers there was room mist labeled "caution" and "keep out of reach of children", and hand sanitizer and several small containers of hand sanitizer and antibacterial hand spray all labeled "warning" and "keep out of reach of children".

Plan of Correction: The facility responded with the following:
All potentially hazardous substances will be kept in locked locations. All staff will be reminded that any substance with a hazard warning must be in a locked place when not in direct use.
