Discussed proof of ID, the vendor does not have to keep copies of the birth certificate but can just fill out the form provided.

The vendor is present with two children at this annual shsi inspection. A total of 6 points.
Observed the children playing, watching a show on TV and getting ready for lunch. The vendor has lots of space and supplies. The children play outside when the weather is nice. The vendor is part of the food program and today the children will have rice, chicken and veggie for lunch with pears and milk.This inspection was started at 10:10 and ended at 11:50. Standard #: 22VAC40-665-160-B Description: Based on the review of the vendor attendance , the vendor failed to record some of the information that would make the attendance record up to date.
Evidence: The vendor does not have the date recorded on the attendance sheet just the month.Plan of Correction: I will add the date to the attendance log.
Standard #: 22VAC40-665-170-B Description: The vendor failed to get some of the information needed to make the child's record complete.
Evidence: Based on the review of 4 records the following items are missing from the children's records:
Child A, B and C are missing proof of ID, Emergency contact information and employment information for parents.
Child A is also missing immunization records.Plan of Correction: I will send all the parents a text message and ask them for the information.
