Comments:
An unannounced monitoring inspection was conducted on 01/25/2023 from 11:45am to 2:25pm. There were 41 children ages 3 years old to age 6 years old supervised by 8 staff. The physical plant, programming, attendance, menus, 3 staff records, 4 children?s records, 2 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed eating lunch and watching a movie. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: During inspection in Class C, there were 13 children present, but the written record reflected there were 16 children present.

Plan of Correction: Violation was addressed and corrected when inspector was present. Violation incurred during transition from morning class to lunch time. Teachers' class rosters reflected students not present.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence: In Class B, there were bleach wipes and a multi-purpose bleach cleaner stored in an unlocked cabinet that was accessible to children.

Plan of Correction: Violation was addressed and corrected when inspector was present. Cleaning supplies were moved to a cabinet out of children's reach. A lock was placed on the unlocked cabinet.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not ensure that proper handwashing procedures were followed.

Evidence:
In Class B, when the last 2 children who were eating finished their lunch, the 2 children were instructed to put away their lunchboxes and join the rest of the children already waiting on the carpet to watch a movie. The 2 children did not wash their hands with soap and running water or disposable wipes after eating lunch.

Plan of Correction: Violation was addressed and corrected when inspector was present. Children were reinstructed to wash their hands. Assistants were reminded to enforce the school policy.
